Computerized Faculty Evaluations Using the NCS Scanner

General Information Summary

Faculty evaluations may be coded on the General Purpose - NCS - Answer Sheet (used for the TEVAL, CTEVAL, or PARVAL programs), the blue #F4162-54321 form used for the BTVAL program), OR on the red CAPS form #F4432-54321 (used for the TECAPS program). The General Purpose - NCS - Answer Sheet is green and white, and is form number 01-30423-3. The answer sheets are available through Office Depot as item numbers 18020 (Red), 18030 (Green), and 18040 (Blue). The instructions listed below must be followed when using these sheets for evaluations.

  • Please do not staple the answer sheets or have other papers mixed in when you submit the forms to Educational and Information Technology. Please put each section in a separate envelope.
  • The first sheet must have the IDENTIFICATION NUMBER section with the four digit course number, and three digit section number - (seven digits) - left justified (i.e., the first digit falling in the leftmost column with no blanks between digits). The appropriate circles must be filled in below the row of boxes. The alpha course prefix of four characters is ignored on the answer sheet.
  • Before submitting the sheets to be processed by Educational and Information Technology, the person administering the evaluation should check to make sure the first sheet has the correct department number, course number, and section number marked in the I. D. section. The instructor's name should then be coded in the NAME section of this first scan sheet. Note that this is not an additional sheet - simply the student response sheet that happens to come first. Fill in the circles at the extreme left of the block, and leave no spaces between the filled-in circles. DO NOT code a zero in front of the department number.
  • Sheets for each individual class should be placed in an envelope and all classes should be submitted at the same time. Please make sure that all sheets run in the same direction and have the same side up. A summary of the classes evaluated will be printed at the end of the output.
  • Use only one type of form at a time. Use either the green, blue, or red form, but do not mix the forms.
  • Fill out the SCANNER REQUEST - FACULTY EVALUATION form (obtained at the I/O area) when submitting sheets to be processed. Fill in the following information: Name Department or College Date Phone No. Evaluation Procedure to be used Number of Questions Responses which should not be used in calculating the mean or standard deviation Report Title Quarter and Year Other options or comments

    NOTE: Omitted questions are not permitted in the analysis.

    Complete ONE Scanner request form for the entire group of the SAME quarter or year, not one for each section.

Each department administering faculty evaluation questionnaires is responsible for preparing a set of evaluation questions on a sheet separate from the green General Purpose - NCS - Answer Sheet or the blue #F4162-54321 form. The red CAPS scanner sheets may be used with either questions on a separate paper OR with the questions printed directly on the CAPS sheets. If you choose to have the questions printed directly on the scanner sheets, a special ink MUST be used. Please contact a member of Data Entry BEFORE you start printing. Using the wrong ink and/or not preparing the master correctly will cause your sheets to be rejected by the scanner.

The evaluation output will be a separate analysis of each section, plus a summary of all sections if requested.

The output report discussed below was generated using SAS (Statistical Analysis System) procedures. Information about the statistical procedures used may be obtained from SAS USER'S GUIDE: BASICS and/or SAS USER'S GUIDE: STATISTICS available at most consulting desks.
