The
GENERAL PURPOSE - NCS - ANSWER SHEET is used for
test scoring at the University of Toledo, form
number 01-30423-3. This form is made for the
scanner and is available from Office Max as item
# D218030. No other form may be used for the general
test scoring program.
The
Identification Number is located on side two of
this form. This ID number is your UT ID # (usually
your SS#). IF THE ID NUMBER IS NOT COMPLETELY
FILLED IN, YOUR OUTPUT CANNOT BE SORTED BY ID
NUMBER. Also on side two is an area for students
to code in their names. This name block can be
filled in for clerical purposes and can be sorted
by name.
All
marks on these sheets should be made using a #2
lead pencil or softer. Erase completely all
errors. Make heavy black marks that fill the
circle completely. Mark one, and only one, answer
for each question. MORE THAN ONE MARK OR LIGHT
MARKS NORMALLY RESULT IN THAT QUESTION BEING
COUNTED AS A WRONG ANSWER.
For
clerical purposes the students' name should be
written on the top of the sheet as indicated on
side one. However, no marks should appear in the
right-hand margin among the black timing marks.
Marks in this area will cause misreading of the
sheet.
The
answer key is provided by marking the correct
answers on a sheet and filling in the ID number
block with all zeros. IT IS VERY IMPORTANT TO CODE
THE ANSWERS WITH HEAVY DARK MARKS AS LIGHT MARKS
WILL NOT BE READ BY THE SCANNER. IF THIS HAPPENS,
THE RUNNING OF THE TEST WILL BE DELAYED UNTIL THE
INSTRUCTOR COMES TO EDUCATIONAL AND INFORMATION
TECHNOLOGY TO FIX THE ANSWER KEY. The characters
that are coded in the name block will be used as a
test label and will be printed at the top of
SECTION I - KEY SHEET. It is suggested that the
instructor's name or course identifier be used.
Please note that any questions on the key that are
to be omitted from the scoring process SHOULD NOT
be bubbled in.
The
answer sheets must be handled fairly carefully so
that they can be properly read by the machine.
This means that they should not be folded or
stapled. The edges should be well protected from
tearing and crumpling.
