Is there a form available that can have the questions printed on the form and is there
an area to have remarks written on the form?
ANSWER:
Yes, it is the red form used for surveys and evaluations. It has room for 50 questions
with 10 responses. It's order number is 18020.
QUESTION:
Can the weighted questions be scattered?
ANSWER:
Yes.
QUESTION:
Can there be more than 1 response to a question?
ANSWER:
Yes. But only bubble in 1 correct response and write all the correct responses on the
back of the request form.
QUESTION:
What is the fee for scanning?
ANSWER:
There is no fee; it is a service.
QUESTION:
What is the turn around time for faculty evaluations, research, etc?
ANSWER:
Normally, 48 hours with the exception of mid-term and finals week. During mid-term and
finals week, they are processed after mid-term and final week are over.
QUESTION:
What is the turn-around time for test scoring?
ANSWER:
Normally, there is 24 hour turn around. However, special requests are exceptions to
the turn around time (ASAP). Ask first to be sure we can.
QUESTION:
If I have an account, what kinds of data can be sent to me?
ANSWER:
We can send you raw data or an output listing (sorted by name, id, or both).

What types of forms are available for scanning?
ANSWER:

Green: used for test scoring, evaluations, surveys, 240 questions and 5
bubbled responses. Order number D218030.

Red: used for surveys and evaluations, 50 questions, 10 responses.

Blue: used for surveys and evaluations, 160 questions, 0-9 bubbled
responses. Order number D218040.

Where do I get forms and what do they cost?
ANSWER:
If your department doesn't have them, they can be ordered through Purchasing from Office
Max. Pricing information can be obtained from either Purchasing or Office
Max. The
red forms are order number D218020, the green forms are
D218030, and the blue forms are D218040.
